Trophy Orders

This form is to order additional CFMA recipient trophies beyond the one trophy provided per category.

The cost of each trophy is: $300 CAD (including shipment to most places in Canada). Note that additional trophies can only be created at certain times of the year. Once you submit your order form, we will be in touch with the approximate timeline for the next batch or orders.

Once this form has been submitted, you will soon receive an email confirming trophy details and approximate timelines. Once a timeline is confirmed, we will send you a PayPal request for payment. You do not need a PayPal account to use your credit card for payment. Shipping is by UPS Ground unless otherwise requested (additional cost may apply). Questions?
